Algemeen gebruik
Nolvadex is een niet-steroïde anti-oestrogeen dat oestrogeenreceptoren in doelorganen en tumoren competitief remt. Het exacte werkingsmechanisme is niet volledig onderzocht, maar het is bekend dat Nolvadex een complex vormt met de receptor en cofactor, dat vervolgens naar de celkern wordt overgebracht. Dit complex voorkomt de hypertrofie van cellen die afhankelijk zijn van oestrogeenregulatie. Het effect van de medicatie kan enkele weken aanhouden na een enkele dosis. Nolvadex kan ovulatie veroorzaken bij vrouwen door de productie van gonadotrope hormonen van de hypofyse te stimuleren. Bij mannen met oligospermie verhoogt het de concentratie van luteïniserend hormoon, follikelstimulerend hormoon, testosteron en oestrogeen in het bloedserum.Nolvadex wordt gebruikt voor de behandeling van borstkanker bij vrouwen en mannen (vooral na castratie), en ook wanneer de kanker is uitgezaaid naar andere delen van het lichaam. Daarnaast wordt het gebruikt bij de behandeling van nierkanker, melanomen, sarcomen van de zachte weefsels met oestrogeenreceptoren en eierstokkanker.
Dosering en gebruiksaanwijzing
Neem Nolvadex precies in zoals aanbevolen door je arts, aangezien de dosering afhankelijk is van je conditie. Een lange behandeling (ongeveer 2 jaar) wordt vaak aangeraden. Neem dit medicijn in met voedsel en een groot glas water. Het kan als een enkele dosis worden ingenomen of verdeeld in twee doses ('s ochtends en 's avonds). Kauw of fijnmaak de tabletten niet.
Voorzorgsmaatregelen
Deze medicatie kan de kans op het ontwikkelen van baarmoederkanker verhogen. Patiënten die worden behandeld met Nolvadex moeten worden gecontroleerd op abnormale vaginale bloedingen, veranderingen in de menstruatie, veranderingen in vaginale afscheiding, of bekkenpijn of -druk.
Contra-indicaties
Nolvadex wordt afgeraden bij patiënten met overgevoeligheid voor dit geneesmiddel en bij vrouwen die een anticoagulantiatherapie van het cumarin-type ondergaan. Het wordt ook afgeraden voor vrouwen met een voorgeschiedenis van diepe veneuze trombose of longembolie, zwangere vrouwen en vrouwen die borstvoeding geven. Voorzichtigheid is geboden bij patiënten met oogaandoeningen (inclusief cataract), hyperlipidemie, leukopenie, trombocytopenie, hypercalciëmie, ernstige tromboflebitis en trombo-embolie (ook in de voorgeschiedenis).
Bijwerkingen
Bijwerkingen worden niet verwacht, maar als ze veranderen van licht naar ernstig, moet je onmiddellijk je arts hierover informeren. Mogelijke bijwerkingen van Nolvadex zijn: opvliegers, misselijkheid, braken, gewichtstoename en abnormale menstruatie.
Interactie met geneesmiddelen
Nolvadex kan afwijkingen veroorzaken in lever- en bloedtesten, dus regelmatige controle op bijwerkingen veroorzaakt door Nolvadex is vereist. Als symptomen zoals geelzucht en bloedingen optreden, moet de patiënt onmiddellijk medische hulp inroepen.
Gemiste dosis
Als je vergeten bent je dosis op tijd in te nemen, doe dat dan zo snel mogelijk zodra je het je herinnert. Neem de gemiste dosis echter niet als het bijna tijd is voor je volgende dosis. Neem geen dubbele of extra doses. Neem je gebruikelijke dosis de volgende dag op het gebruikelijke tijdstip in.
Overdosis
Acute overdosering bij mensen is niet gemeld. Bij patiënten met uitgezaaide kanker die werden behandeld met de maximaal getolereerde doses Nolvadex, werd acute neurotoxiciteit waargenomen, gekenmerkt door tremor, hyperreflexie, onvast lopen en duizeligheid. Als je vermoedt dat je te veel van deze medicatie hebt ingenomen, zoek dan onmiddellijk medische hulp en neem contact op met je arts.
Bewaarvoorschriften
Bewaar op kamertemperatuur tussen 20-25°C (68-77°F). Houd de medicatie verwijderd van vocht, warmte en direct zonlicht.

I was taking Nolvadex 20 mg to prevent the recurrence of estrogen-positive tumors, the treatment period was 5 years. I got the results back not so long ago, and it's all good. I got so used to taking Nolvadex 20 mg during this time, I think it will be tough not taking it. I was taking one pill a day and had some menopause symptoms, which I guess is unavoidable when you are taking a drug to suppress estrogen.
